Got back to the rear throttle again tonight for a short session.
I installed the throttle lever temporarily and determined where I was going to connect the rod end. I then marked the height I wanted to cut the lever.
I was actually able to invert the throttle lever and use the hole that is meant for the rod end bearing attachment (which I couldn't use anyway due to my throttle location) so I cut the lever just above that location and fit the knob to the top of the lever. The lever took some grinding and scotch brite work to get the knob to fit.
I marked the location of the rod end bearing and drilled it. I slipped a bolt in the rod end and lever and checked it to see if everything was in the ballpark, which is was. I have to tweak the rod bends a little and put the nylon guides in place for the lever but it is almost done. I also have to bend the throttle just a little towards the inside of the airplane.
